Street and house number of your permanent residential address. Pay attention to uppercase/lowercase letters and spaces. Your address will not be made public – this information is for our use only .
City of your permanent residence.
ZIP code of your permanent residence city.
Select your country.
Your phone number  where you are normally reachable. We will not publish your phone number – this information is strictly for our internal use (for exceptional situations that must be resolved immediately and cannot be delayed). Include the international prefix (eg. +420 123 456 789).
Bank account number  for royalty payouts . It must be in the Czech format "account number / bank code" (e.g., 1234567890/0800) or "prefix - account number / bank code" (e.g., 250000-1234567890/0800), or alternatively in the international IBAN format (e.g., CZ0000000000000000000000) - however, we strongly recommend Czech banks .

I DON'T HAVE a Spotify (for Artists) or Apple Music (for Artists) artist account yet = I AM a new artist.
I ALREADY HAVE a Spotify (for Artists) or Apple Music (for Artists) artist account = I AM NOT a new artist.
Since you checked above that you already have a profile on Spotify/Apple Music, you must enter YOUR Spotify URI in the format: spotify:artist:1HY2Jd0NmPuamShAr6KMms. Here is a guide and a handy tool to find your Spotify URI.  Be careful. If you enter the Spotify URI incorrectly, your music will be published to a new or someone else's artist account. If you don't have an artist account on Spotify, change your selection above.

Since you checked above that you already have a profile on Spotify/Apple Music, you must enter YOUR Apple Music ID in the format: 6 to 15 digits. Here is a guide and a handy tool to find your Spotify URI.  Be careful. If you enter the Apple Music ID incorrectly, your music will be published to a new or someone else's artist account. If you don't have an artist account on Apple Music, change your selection above.
